Job Description

To provide medical care to residents of the nursing home, together with other multidisciplinary care team, focusing on the prevention, diagnosis, and treatment of illnesses, management of chronic conditions, and ensuring residents' overall well-being.

Job Responsibilities

1. Medical Care

  • Conduct initial medical assessments for new admission cases within 48 hours of admission.
  • Develop and maintain individualized care plans in collaboration with nursing, physiotherapy, and any other allied health teams.
  • Conduct consultations to assess, diagnose, and treat a wide range of medical (acute and chronic) conditions.
  • Coordinate and monitor referrals to other healthcare providers as needed.
  • Maintain accurate and timely medical documentations in compliance with regulatory standards.

2. Preventive & Palliative Care

  • Oversee preventive health programs including vaccination, fall prevention and nutrition.
  • Provide palliative and end-of-life care with sensitivity, respecting resident wishes and advance directives.
  • Ensure wound / pain management and symptom control for residents with terminal or advanced chronic illnesses.

3. Emergency Response

  • Respond promptly to medical emergencies within the facility.
  • Liaise with emergency services and hospitals when transfer is required.
  • Participate in post-incident reviews to improve response protocols.

4. Coordination & Communication

  • Work closely with the nursing team, allied health professionals to ensure holistic person-centred care
  • Participate in interdisciplinary team (IDT) meetings to review residents' progress.
  • Communicate with residents' families/sponsors, providing updates and addressing concerns or change in residents' medical condition when necessary.
  • Attend family conference when needed.
  • Facilitate advance care planning (ACP) and preferred plan of care (PPC) conversations with residents and family members/sponsors.

5. Compliance & Quality Assurance

  • Ensure compliance with relevant health regulations, licensing requirements, and infection control policies
  • Ensure medical documentation is accurate, timely and compliant with regulatory requirements.
  • Participate in clinical audits, quality improvement initiatives, morbidity/mortality reviews and incident reporting.
  • Stay updated on nursing home/geriatric care best practices and emerging medical guidelines.
6. Role in Infection Control and Prevention Program
  • Participate and involve in supporting and playing advisory/consultancy role in the Infection Control and Prevention Program
  • Contribute in the writing of policies on antibiotic prescribing.
  • Review the appropriateness of antimicrobial at or after 48 hours from initial orders
  • Implement practices to improve the use of antibiotics.
Qualifications
  • MBBS degree from a recognized medical institution.
  • Certification or experience in geriatric or family medicine would be an added advantage.
  • Full registration with Singapore Medical Council (SMC)
  • Have the relevant knowledge inInterRAI and Resident Assessment Forms (RAF) would be an added advantage.
Technical Skills

Work Schedule
