The Rehabilitation Specialist supports seniors across different levels of frailty by designing, implementing and monitoring personalised care plans within an integrated wellness model. Working alongside a medical coach, fitness coach, and nutritionist, the role ensures seniors receive holistic support in physical, cognitive, and functional wellness. The Specialist plays a key role in delivering safe, evidence-based rehabilitation programmes within the Active Ageing Centre gym and community setting.
Job Description:
a) Personalised Care & Intervention
- Conduct functional assessments (e.g., balance, strength, mobility) to determine seniors’ needs and frailty levels.
- Co-develop personalised care plans with the medical coach, fitness coach and nutritionist.
- Deliver customised rehabilitation sessions targeting strength, balance, mobility, cognitive stimulation, and activities of daily living (ADLs).
- Monitor progress and adjust interventions in response to seniors’ health status and goals.
b) Programme Delivery & Safety
- Lead or support structured gym-based and group exercise sessions for seniors of varying capabilities.
- Ensure safe use of equipment and adherence to gym protocols.
- Provide education to seniors and caregivers on functional maintenance and fall prevention
c) Digital Tracking & Data Management
- Use digital tools to record assessments, track progress, and generate care insights.
- Monitor development using dashboards, wearable data, and Active Ageing Centre digital systems.
- Ensure accurate documentation for audit, evaluation, and MOH reporting requirements.
d) Research & Quality Improvement
- Collaborate with research teams to pilot, evaluate, and improve care models and rehabilitation protocols.
- Support outcome measurement, data collection and analysis.
- Participate in continuous improvement projects aligned with Active Ageing Centre and organisational goals.
e) Interdisciplinary Collaboration
- Work closely with the medical coach, fitness coach and nutritionist to ensure integrated, holistic care.
- Attend interdisciplinary meetings to discuss seniors’ progress and care adjustments.
- Support health education activities, workshops, and community outreach.
f) Senior Engagement & Empowerment
- Motivate and guide seniors to adopt sustainable, healthy lifestyle habits.
- Support seniors transitioning towards self-management and independence.
- Promote participation in Active Ageing Centre programmes to enhance social connectedness.
Job Requirements:
- Diploma or Degree in Physiotherapy, Occupational Therapy, Exercise Science, Gerontology or equivalent.
- Preferably registered with AHPC (if holding clinical qualification).
- Experience working with seniors in community, rehabilitation or gym settings is preferred.
- Strong communication skills with ability to engage seniors of diverse backgrounds.
- Comfortable with digital tools, data collection, and basic analytics.
- Passionate about active ageing, rehabilitation and community wellness.
