Operating under an integrated care setting, you play a pivotal role in providing primary nursing care and management to our clients. You will support clinical administrative duties and provide supervision, guidance, and direction to juniors to ensure that optimal care is provided to meet the desired positive client outcomes/experiences.

Key responsibility areas
  • Develop care & health plans according to the assessed clients’ needs
  • Evaluate, monitor and document clients’ progress according to planned care
  • Ensure continuity of client care and treatment plans through follow-ups
  • Administer vaccinations and medications (when necessary)
  • Educate clients/ family members about preventive care, nutrition, fitness and hygiene.
  • Communicate clients’ progress with client/ & family.
  • Facilitate health related workshops (when necessary)
  • Collaborate with other healthcare professionals to provide comprehensive health care services.
  • Respond to health emergencies.
  • Advocate for health policies/ information that benefit the clients/community, i.e. Advanced care planning, dementia prevention, therapy education and rehabilitation techniques.
  • Supervise and guide nursing students and care assistants in their work.
  • Participate in clinical/service quality improvement and activities and provide suggestions to improve workflow.
  • Perform other duties as assigned by the Centre Manager/Management
Required qualifications and experience
  • At least a Diploma in Nursing or Diploma in Health Sciences (Nursing).
  • Registered Nurse with the Singapore Nursing Board (SNB), current nursing licensure in the state of practice.
  • Minimum 2 years or above working experience with similar settings.
  • Able to converse in simple local languages and dialects.
  • Reliable team member, possess positive work and good interpersonal skills.
  • Ability to work independently and as part of a multidisciplinary team.
  • Compassionate, positive and possess a caring attitude towards clients.
  • Good team player, and be knowledgeable.
  • Resourceful, systematic, and highly motivated.
