Job summary

Responsibilities

Job description
Job Scope
  1. Plan and perform outreach programs and interventions
  2. Conceptualize, propose and implement programs and activities to reach out or attract target groups
  3. Engage and manage volunteers in conducting programs and mentoring youths
  4. Cultivate a support network and build working relationships with relevant agencies and volunteers to ensure success of programs
  5. Facilitate invention work for youths identified to be at-risk
  6. Build trust and instill positive values among youths
  7. Engage, mentor and counsel alienated and at-risk youths who reject formal activities so as to build rapport for future intervention
  8. Develop and maintain a trusting relationship with youths
  9. Build and engage social networks to identify opportunities for social services targeting youths in particular
  10. Create opportunities for mentoring, coaching and supporting youths to facilitate their personal, social and educational growth
  11. Keep proper records, tracking, monitoring and evaluation systems
  12. To provide feedback to Social Workers on progress of residents in their therapeutic goals and liaise closely with Care Staff team regarding program schedules
  13. To ensure that the residents are punctual and on time for activities and programs
  14. To treat every resident with dignity, respect and equality
  15. As a team member, you are to be available at all times (whether on or off duty) to assist colleagues in need
  16. To work closely with and report to Head of Home / Head (Therapy)
  17. To abide by the Code of Conduct
  18. To accompany the residents for activities when required
  19. Be vigilant to the resident’s mood and/or behavioral changes
  20. To ensure every resident adheres to rules and regulations
  21. To undertake any other duties specified by Head of Home and/or senior management
