A leading organization in the healthcare sector, dedicated to supporting patients' smooth transition from hospital to home and strengthening care networks, is seeking an experienced Principal Care Coordinator to lead and develop the care coordination team.

The Role
  • Lead a team of Care Coordinators providing post-discharge care coordination and case management services to reduce unnecessary hospital readmissions.
  • Develop, review and implement policies, procedures and initiatives to improve patient and caregiver support.
  • Strengthen and advance frameworks and processes for the community care ecosystem.
  • Oversee follow-ups (phone calls, home visits), data management, and reporting.
  • Collaborate with acute care and community partners to ensure seamless care continuity.
  • Provide clinical supervision, manage staff performance and plan professional development.
The Talent
  • Professional qualification in Nursing, Social Work, Allied Health, or a relevant clinical discipline.
  • At least 8 years' experience in care coordination, case management and/or clinical reasoning and post discharge care.
  • Experience in leading a team.
  • Strong leadership, communication, and stakeholder management skills.
