Responsibilities

• Provide safe and efficient delivery of pharmacy services.
• Review medication orders for completeness and appropriateness.
• Process drug orders and dispense medications to patients.
• Counsel patients on their medications.
• Perform medication reconciliation.
• Provide drug information to healthcare professionals.
• Assist in preparation of ad-hoc pharmacy items.

The Role

Provide safe and efficient delivery of pharmacy services (dispensing/counseling) in the hospital.

Key accountabilities

• Review medication orders for completeness and appropriateness
• Process drug orders and dispense medications to patients
• Counsel patients (inpatients / outpatients) on their medications and provide specialised counselling if needed
• Perform medication reconciliation
• Provide drug information to doctors, nurses and patients
• Assist in the preparation of ad-hoc items e.g. extemporaneous preparations, reconstituted antibiotic suspensions
• Other duties as assigned
