We are seeking a dedicated and skilled healthcare professional to provide high-quality patient care and guidance in a dynamic nursing environment. This position offers a unique opportunity to mentor colleagues and contribute to the development of nursing practices in home-based and palliative care settings.

Responsibilities

• Deliver independent clinical assessments and develop personalized care plans with input from the multi‑disciplinary team, ensuring appropriate referrals.
• Offer mentorship to junior nurses in managing complex care needs of patients.
• Provide after‑hours phone support to patients and caregivers on a scheduled basis.
• Develop and implement orientation and preceptorship programs for new staff.
• Coordinate ongoing training and competency assessments for nursing staff.
• Supervise junior nurses and nurse managers, ensuring operational needs are met.
• Lead initiatives aimed at promoting the well‑being of nursing staff.
• Establish clinical audit frameworks and conduct audits to uphold nursing standards.
• Drive quality improvement projects and support policy review and implementation.

Requirements

• Degree in Nursing and/or Specialist/Advanced Diploma in Palliative Care, Gerontology, or a related field.
• Registration with the Singapore Nursing Board.
• Proven experience in nursing with a focus on home-based and palliative care.
• Strong mentorship and leadership abilities.Excellent communication and interpersonal skills.
• Capacity to manage complex clinical cases effectively.
