Qualifications

  • Fresh graduate will be considered.
  • At least 1 year of relevant healthcare industry experience.
  • Linguistic capability in English, Mandarin, and local dialects advantageous.

Job description

The Senior /Medical Social Worker Assistant supports and assists the Senior / Medical Social Worker (S/MSW) to look into the possible interventions (to receive social and financial assistance, renew subvention of healthcare subsidy and eligible funding schemes) to help reduce the burden of the clients and families from incurred social and healthcare costs.

Work Location: Bukit Batok / Woodlands / Tampines Care Home

Responsibilities
  • Regular liaisons with clients and families to following up on pending assistance – Medifund, Household Means Testing (Subvention) and other funding schemes, etc.
  • Be updated with available financial assistance available and provide advice to clients and their families on eligible resources to tap on
  • Support and adhere to the care plans developed by S/MSW (Centre Manager) and other healthcare colleagues
  • Monitor clients’ social wellness and participation rate in social activities during their stay
  • Assist in follow-up and liaisons with community services in the discharge care plan of clients, if any.
  • Financial counselling for all clients during assessment
  • Closely monitor to ensure the meeting of KPI of 10 days for referral intake
  • Provide administrative support by keeping records, filing and update progress reports, etc.
  • Perform any other duty as assigned by the Centre Manger or Reporting Manager
Requirements
  • Diploma in Social Work
  • Fresh graduate will be considered as well
  • At least 1 year of relevant healthcare industry experience
  • Pleasant and mature disposition with empathy and patience in liaisons with everyone
  • Demonstrate initiatives, good communications and people engagement skills
  • Linguistic capability in English, Mandarin and advantageous with the use of local dialects to interact with the local clients
  • Good organisation ability
