Work Location: Bukit Batok Care Home / Woodlands Care Home / Tampines Care Home

The Senior / Medical Social Worker Assistant supports and assists the Senior / Medical Social Worker (S/MSW) to look into the possible interventions (to receive social and financial assistance, renew subvention of healthcare subsidy and eligible funding schemes) to help reduce the burden of the clients and families from incurred social and healthcare costs.

Responsibilities
  • Regular liaisons with clients and families to following up on pending assistance – Medifund, Household Means Testing (Subvention) and other funding schemes, etc.
  • Be updated with financial assistance schemes available and provide advice to clients and their families on eligible resources to tap on.
  • Support and adhere to the care plans developed by S/MSW (Centre. MSW Manager) and other healthcare colleagues.
  • Monitor resident social wellness and participation rate in social activities during their stay.
  • Assist in follow-up and liaisons with community services in the discharge care plan of residents, if any.
  • Financial counselling for all clients during assessment.
  • Provide administrative support by keeping records, filing and updating progress reports, etc.
  • Prepare necessary documentations for Audits.
  • Perform any other duty as assigned by the Centre Manager or Reporting Manager.
Requirements
  • Entry level candidates will be considered.
  • Preferably with at least 1 year of relevant healthcare industry experience.
  • Pleasant and mature disposition, possess empathy and patience dealing with residents, next of kins, and staff alike.
  • Demonstrate initiative to identify, plan, develop and implement resourcefulness to find solutions to missing gaps in work processes and mutually benefit stakeholders.
  • Good communication and people engagement skills.
  • Good organization ability.
  • Able to form networks and strong linkages with stakeholders.
