Job summary

SG REHAB PTE. LTD. seeks a Locum Occupational Therapist to support home therapy for clients in the West and Central areas of Singapore. This part-time role involves visiting clients' homes and delivering person-centred therapy.

Start date: October 2026. Duration: One year with possible extension. You will assess ADLs, develop intervention plans, train caregivers, and document progress while coordinating with the referring organisation.

Qualifications

• Degree or Diploma in Occupational Therapy.
• Full AHPC Registration.
• Relevant clinical experience, preferably in community rehabilitation or home-based care.
• Able to work independently and manage home visits.
• Willing to travel to clients’ homes in the West and Central areas.

Responsibilities

• Conduct occupational therapy assessments in clients' homes.
• Develop individualised intervention plans based on each client’s needs and goals.
• Provide therapy to improve clients’ independence, safety and participation in daily activities.
• Assess and provide recommendations on activities of daily living, mobility, transfers, and home safety.
• Recommend suitable assistive devices, adaptive equipment or home modifications where required.
• Educate and train clients, family members and caregivers on therapy activities and safe care techniques.
• Monitor clients’ progress and adjust intervention plans accordingly.
• Maintain accurate and timely clinical documentation.
• Communicate and coordinate with the referring organisation and other relevant care professionals.

• Provide appropriate therapy to improve clients’ independence, safety and participation in daily activities
• Assess and provide recommendations on activities of daily living, mobility, transfers, positioning and home safety
• Recommend suitable assistive devices, adaptive equipment or home modifications where required
• Educate and train clients, family members and caregivers on therapy activities and safe care techniques
• Monitor clients’ progress and adjust intervention plans accordingly
