Responsibilities

• Assess patient’s nutritional needs and provide basic nutrition education.
• Teach tests for monitoring chronic diseases.
• Track patient’s progress and lifestyle changes.
• Manage complex chronic cases with possible home visits.
• Conduct Diabetic Foot Screening and referrals.
• Coordinate care with Primary Care Coordinators.
• Participate in meetings and case discussions.
• Support doctors in clinical activities as needed.
