SPD Singapore seeks a Social Worker to deliver intervention and case management for children and families in the Early Intervention Programme for Infants and Children (EIPIC). You will collaborate with EI professionals in a transdisciplinary team, conduct intake and needs assessments, guide families through orientation, and identify risks and protective factors for tailored interventions.

You will support transitions, coordinate with parents and external partners for SPED or mainstream school

Competenze

• Bachelor of Social Work and RS(W) registration is required.
• Experience in disability or community-based services is preferred.
• Strong analytical and clinical reasoning skills are essential.
• Ability to work effectively in transdisciplinary teams.

Mansioni

• Conduct intake and needs assessments and develop intervention plans.
• Provide trauma-informed, person-centred psycho-emotional support to clients and caregivers.
• Coordinate referrals to services and community resources.
• Lead community outreach and capacity-building initiatives.
