Job Description The Health Care Assistant is responsible for the delivery of daily clinical services to clients under the Home Care Services (HCS). The Health Care Assistant will act professionally at all times and be accountable to the client and the organization to ensure that care is delivered in a safe and timely manner.

Job Responsibilities

• Render care to clients in accordance with the Community Care Skills Standards Framework
• Provide direct Home Care Services (HCS) to clients
• Perform basic skills such as:
• Vital signs monitoring
• Assistance with Activities of Daily Living (ADL)
• Assistance with Instrumental Activities of Daily Living (iADL)
• Provide medication assistance/reminder
• Escorting of clients
• Transfer and use of mobility aid
• Conduct appropriate exercise and activities
• Provide service delivery according to service contract and maintain documentation accordingly
• Be a good team player, building positive influence to team and motivated to learn and grow towards self-excellence
• Participate in outreach and related activities
• Communicate and relate effectively at the workplace
• Perform any other duties as directed by supervisor and/or management
