Job summary

Qualifications

• Must hold a nursing degree with at least 5 years of perm inpatient experience.
• Advances in geriatric / palliative care are desirable.

Responsibilities

• Implements patient care in collaboration with MDT per assessed needs.
• Facilitates admissions, transfers and discharges with timely discharge planning.
• Monitors patient progress and escalates deterioration as needed.
• Leads and participates in MDT rounds and follows physicians’ orders promptly.
• Assists with bedside procedures and treatments.
• Educates patients and families on disease progression and self-care.
• Ensures infection control practices and optimizes resource use.
• Supervises nursing staff and provides feedback.
