Purpose of job
- Assess the care needs of clients and caregivers living with mental illness and develop individualised care plans and interventions.
- Work collaboratively with healthcare professionals, community partners and families to provide holistic, person-centered care.
- Provide supervision and guidance to junior staff in managing casework and support services.
Job Description
- Manage and coordinate COMIT post-discharge programmes in partnership with tertiary healthcare institutions, including monitoring clients’ progress and programme outcomes.
- Conduct client and caregiver assessments using the Bio-Psycho-Social-Spiritual (BPSS) model to develop and implement individualized care plans and interventions.
- Deliver psychotherapy and conduct mental health talks for clients and caregivers.
- Build and maintain strong networks with stakeholders and community partners to address service gaps and improve care accessibility.
- Supervise the COMIT team through individual and group supervision sessions.
- Promote ongoing training and professional development of team members, aligned with AIC Mental Health Competency Framework.
- Plan, facilitate and evaluate group work sessions for clients and caregivers.
- To undertake any duties as assigned by the Head, Community Mental Health and the Executive Director.
Requirements
- 3 to 5 years of relevant experience in mental health and dementia care, with demonstrated competencies in case management, group work, programme development, and community engagement.
- Graduate Diploma, Bachelor's, or Master's degree in Social Work, or Nursing or Relevant Allied Health Qualifications
- Able to travel and conduct home visits within the designated service boundary.
- Experience with case management practices for individuals with mental illnesses and dementia.
