What the role is:
You will join the Population Health Division (PHD), responsible for developing strategies, concepts, and plans to improve long-term population health. You will work in a fast-paced, dynamic environment that requires managing multiple priorities and stakeholders simultaneously.
What you will be working on:
You will lead a team to:
- Formulate, review, and implement strategies and plans based on evidence and research to identify target population segments and enhance their health outcomes.
- Review and improve selected resident service journeys for key population segments in collaboration with partners, aiming to increase uptake of preventive health measures, such as the Healthier SG enrolment and health plan journey.
- Guide the development of resident-facing IT solutions to meet population health objectives, including supporting behavioral change and scalability.
- Organize and oversee the evaluation of population health initiatives and determine subsequent actions based on findings.
- Collaborate with partners to implement and monitor these efforts.
