Responsibilities
- Provide structured nurse counselling for chronic disease management.
- Conduct screenings and track patient progress through consultations.
- Collaborate with care coordinators and healthcare professionals.
- Provide structured nurse counselling using motivational interview, including education on the conditions of the different chronic diseases, chronic disease management, as well as health promotion and prevention
- Provide case management of complex chronic disease cases which involves a comprehensive care approach to support individuals and/or their caregivers living with chronic health conditions
- Teach skill specific tests for monitoring of patient’s progress (e.g., blood glucose, blood pressure, etc.) and track patients’ progress via telephone consultation on their chronic disease management and lifestyle changes.
- Conduct Diabetic Foot Screening and Diabetic Retinal Photography, and spirometry services (if required). Provide education and referrals of “At Risk” cases based on PCN’s protocol and in consultation with the doctor.
- Raise “priority risk” cases for case discussion and raise cases to the doctors based on PCN’s escalation protocol.
- Participate in developing structured programs / care plans for good management and improvement of care.
- Participate in PCN meetings, CME talks and case discussions and provide inputs on clinical results of the various clinics in the Primary Care Network.
- Work closely with the Primary Care Coordinators in care coordination and care transition. This involves liaising with the public institutions, clinic’s doctors and assistants, community health partners and social providers.
- Collaborate with the management team to identify areas for improvement and implement strategies to enhance the quality of patient care.
- Identify learning needs, develop and oversee the development of training plans for nurses and clinical primary care coordinators to enhance their skills and knowledge.
- Develop competency and re-assessments protocol for the nurses and primary care coordinators based on the competency assessment guidelines.
- Review or revise nursing guidelines (including escalation protocols), standards and procedures based on MOH guidelines.
- Supervise and mentor newly hired nurses by providing guidance, support, and constructive feedback.
- Perform routine review of nurse counsellors and clinical care coordinators' work to ensure adherence to established protocols and guidelines.
- Support other tasks that arise from the development of new initiatives (e.g. Healthier SG) delegated by reporting officer.
