Qualifications

  • Degree or higher in Social Work required.
  • Preferred accreditation with the Social Work Accreditation Board.

Responsibilities

  • Conduct holistic bio-psychosocial-spiritual assessments.
  • Provide counselling and psychoeducation to patients and families.
  • Develop individualized care plans with health/social care teams.
  • Actively participate in interdisciplinary team meetings.
  • Assess risk factors and develop safety strategies.
  • Facilitate community group work programmes.
  • Collaborate with health and social care providers to join up care.
  • Involvement in departmental projects and initiatives.

Education

Degree/Post-Graduate Diploma in Social Work
Accreditation with the Social Work Accreditation Board (SWAB) is preferred
