Job summary

A leading social service provider in Singapore is seeking a Case Manager (Social Worker) to join their COMIT Team. The role involves overseeing the triaging process, case management, and collaboration with community agencies to facilitate effective service delivery. Candidates should hold a Bachelor's degree in Social Work and possess at least 3 years’ experience in case management, ideally within the healthcare or social service sector. Strong interpersonal skills and supervisory experience are preferred.

Qualifications

• Minimum of 3 years of working experience with case management, preferably in the healthcare or social service sector.
• Experience in social service sectors or familiarity with the ASCAT/COMIT Programme is an advantage.

Responsibilities

• Assist with the triaging process for the AIC COMIT Programme.
• Ensure programme data and documents are maintained according to COMIT requirements.
• Participate in meetings with key stakeholders to share learning points.
• Network with community partners for collaboration and case co-management.
