A leading social service provider in Singapore is seeking a Case Manager (Social Worker) to join their COMIT Team. The role involves overseeing the triaging process, case management, and collaboration with community agencies to facilitate effective service delivery. Candidates should hold a Bachelor's degree in Social Work and possess at least 3 years’ experience in case management, ideally within the healthcare or social service sector. Strong interpersonal skills and supervisory experience are preferred.
The Case Manager (Social Worker) will be part of the COMIT Team who will ensure service delivery is in accordance to the intended programme objectives and care model for the programme as outlined under the terms & conditions of the Letter of Award/Addendum.
The Case Manager will work closely with the Case Integration Lead to oversee the triaging process, reviews and work towards achieving the COMIT goals of timely assessment, provision of intervention and support eg. psychoeducation, case management and counselling, as well as caregiver support and training.
In addition, the Case Manager (Social Worker) may also be involved in networking with other community partners or agencies, for purpose of collaboration and to facilitate co-management of cases where applicable and other adhoc duties assigned.
This candidate will be one of the key members driving the COMIT project.
