A community support organization in Singapore is seeking a candidate to provide case management services, focusing on vulnerable individuals in the community. The role involves developing care plans, managing case documentation, and participating in community outreach initiatives to promote well-being. A Bachelor's Degree in a relevant field is required, along with organizational and interpersonal skills. This position offers an opportunity to make a meaningful impact on individuals' lives.
The incumbent will carry out case management work and apply intervention strategies appropriately, under the guidance of the supervisor, to resolve presenting issues and improve the well‑being of clients. He/ She will also be involved in the centre’s community outreach and public education initiatives as well as support the centre’s volunteer management initiatives.
Providesa person‑centered case management services to vulnerable individuals living in the community which includes holistic intake assessment, individualized care plans, home visit, basic counselling, care coordination and monitoring, service linkage, and other clinical best practices relevant to promoting clients’ wellbeing.
Work with volunteers to provide befriending and support to clients.
Provide basic emotional and support services to caregivers.
Manage and maintain case progress notes, home visit logs, case documentations, and case statistics (when necessary) in a timely and effective manner that complies with regulatory reporting standard.
Develop, review, and update appropriate intervention strategies in consultation with supervisor.
Participate in case reviews, case supervisions, and case conferences.
Participate in the care network and coordination with stakeholders within centre’s service boundaries.
Assist in the outreach initiatives such as community mental health screening, health talk (when necessary), and promoting centre’s services to create public awareness of healthy aging.
Perform other duties assigned by the Reporting Officer.
Bachelor’s Degree preferably in Social Work, Psychology, Sociology, Gerontology or Counselling
2 – 3 years relevant experience, candidates without experience are welcome to apply
Have a passion to work in a community‑based setting; possess good knowledge of healthcare and social services.
Have good organizational skills, is team‑oriented with excellent interpersonal and communication skills; be resourceful and collaborative.
