Job summary

A leading mental health organization in Singapore seeks a case manager for individuals aged 13-40. Responsibilities include coordinating care, conducting assessments, and developing psychosocial intervention plans. Candidates must have a degree in Social Work or Psychology and experience with young persons or psychiatric disorders is preferred. Job description
Overview

To be responsible for the provision of case management services for individuals aged 13 – 40 years old, suffering from early psychosis or at risk of developing psychosis. The successful candidate will be involved in the following areas:

Clinical
  • Provide and coordinate continuity of inpatient and outpatient care from initial consultation through to recovery
  • Conduct assessment of patients for medical, psychological, social, environmental, vocational and financial needs
  • Formulate and develop psychosocial intervention plan with patient and / or caregiver
  • Implement intervention plan and monitor patient’s progress
  • Review and evaluate effectiveness of intervention plan at appropriate juncture with patient and treating team
  • Provide supportive therapy and psychoeducation to patient and caregiver
  • Conduct risk assessment and management proficiently
  • Work in close collaboration with multi-disciplinary team and community partners
  • Conduct group work with patients and/or caregivers
Research
  • Conduct research ratings with patients and/or caregivers
The Requirements
  • Degree in Social Work or Psychology preferred
  • Working experience with young persons, or individuals suffering from psychiatric disorders will be of added advantage
  • Must be a team player with passion in mental health recovery, strong communication skills and willingness to grow professionally with the organisation
