Job description
Job Purpose

The successful incumbent will assist the Assistant Director (Programme)/ Lead Case Manager in the execution and oversight of the programming aspect for Residents emplaced at the HWH and ensure that all programmes carried out are in line with the philosophy of the HWH as well as its mission, vision and core values

Major Duties and Responsibilities
  • Assist the Assistant Director (Programme) / Lead Case Manager to deliver relevant rehabilitation and reintegration programmes to ensure that Residents are kept meaningfully engaged during their stay at the HWH.
  • Monitor and maintain programme integrity so as to ensure that programmes/sessions are delivered to achieve the best intended outcome.
  • Provide case management to Residents assigned under their care. This includes assisting Residents in working out their needs and providing timely and relevant Information & Referral (I&R) to link Residents and their families to appropriate services and assistance.
  • Discuss and work with Residents on their accommodation, conduct and employment matters during HWH phase. This includes conducting home visits (when applicable), job site visits and monthly employment checks with employers to follow up on the Residents’ performance at work.
  • Conduct and facilitate group psychology-based correctional programme(s) with Residents undergoing the Community Based Programme (CBP) at the HWH.
  • Plan, organise and implement (and/or guide Residents to carry out) outreach activities and events. This involves, but not limited to, working with programme providers and managing volunteers.
  • Responsible for Residents’ safety when Residents are engaged in group activities and events organised by the HWH.
  • Track and evaluate the Residents’ progress during their stay at the HWH. This includes involvement in case discussions among the HWH Programme Unit and case conferences with the relevant SPS officers.
  • Manage Residents’ case files and ensure that all case information and relevant documents are completed and updated regularly.
  • Compile relevant details and provide timely updates (e.g. Residents’ progress update and attendance for sessions/activities and events etc.) for various reporting and submission purposes.
Any Other Duties

Carry out any other duties as assigned by the Assistant Director (Programme) / Lead Case Manager.

Job Requirements
  • Recognised Degree in Social Sciences such as Social Work or Psychology, or other related disciplines.
  • Preferably at least three (3) years of working experience, preferably in the social service sector.
  • Team player with good interpersonal, oral and written communication skills.
  • Counselling related certification would be an advantage.
  • Proficient in Microsoft Office applications.
  • Able to multi-task, work independently and as a team player.
  • Have passion in the helping profession.
  • Level-headed and able to work with varying resident profiles
  • Keen learner who is willing to work beyond boundaries.
