Working Location: North East, Hougang

Key Responsibilities

• Assist Centre Manager/Medical Social Worker/Psychologist in conducting client assessments and developing care plans.
• Support clients and caregivers with psychosocial interventions and community resources.
• Coordinate referrals and follow-ups with healthcare providers and social service agencies.
• Participate in home visits, community outreach, and multidisciplinary team meetings.
• Maintain accurate documentation and contribute to service audits and reviews.

Requirements

• Diploma or Degree in Social Work, Psychology, Nursing, Counselling, or related fields.
• Registration with relevant professional bodies (e.g., Singapore Nursing Board, Allied Health Professionals Council, Singapore Association of Social Workers) would be preferred.
• Minimum 1–3 years of experience in mental health, geriatric, or community care settings.
• Familiarity with psychiatric rehabilitation principles is advantageous.
