Job Description
  • Assess clients’ needs: Meet with clients to understand their medical, physical, emotional, and social needs. Use tools like interRAI, MBI, and AMT, and review medical records or reports from other professionals.
  • Plan and coordinate care: Work with doctors, nurses, therapists, and other team members to create care plans that meet each client’s needs. Identify any urgent health or social issues and take action to address them.
  • Support and educate clients and caregivers: Teach clients and their families how to manage their health and daily living needs. Encourage independence and help them make informed choices about their care.
  • Connect clients to services: Act as the main point of contact between clients, caregivers, healthcare providers, and community organizations. Help them access the right services, financial aid, and support programs.
  • Promote government programs: Share information about available government health and social care schemes, and help clients benefit from them.
  • Improve processes: Suggest and support improvements to how care transitions and client services are managed. Serve as a subject matter expert in care coordination.
  • Other duties: Take on any additional tasks as assigned.
Requirement
  • Degree/Diploma in Nursing, Health Science, Social Work, Health Promotion, Health Services Management, or a related/equivalent discipline.
