Job summary

A healthcare service provider in Singapore is seeking a Care Manager to oversee patient care management. Key responsibilities include identifying care needs, monitoring patient progress, and engaging with families to ensure satisfaction. Ideal candidates will hold a Diploma or Degree in Nursing with valid registration, and an Advanced Diploma is a plus. This position includes a competitive remuneration package and office hours of 5.5 days.

Qualifications

  • At least Diploma / Degree in Nursing with valid SNB registration.
  • Candidate with Advance Diploma in specialty area will be a plus.

Responsibilities

  • Identify and assist patients who require care management plan.
  • Monitor residents’ daily schedule and care plan.
  • Monitor and track patient's progression to ensure optimal outcomes.
  • Engage with residents and families regarding quality of care satisfaction.
  • Work with clinical team and stakeholders for quality resident care.
  • Document and ensure confidentiality of patient's records.
  • Participate in quality improvement projects.
  • Perform/participate in outreach programs or activities.
  • Any other duties required within the team.

  • Engage with residents and family members to determine satisfaction with quality of care
  • Work with clinical team and stakeholders to provide quality care to the resident
  • Documenting and ensure confidentiality of patient's records
  • Participate in quality improvement projects
  • Perform / participate in outreach programs or activities
  • Any other duties required within the team
