Job description

Care Coordinator Associate, PHIC - Community Nursing

The incumbent will work under the guidance and direction of a Community Nurse to coordinate and navigate the care of residents who will keep well, get well and age well in the community. You will assist the Community Nurse to coordinate all aspects of health and social care, and act as a liaison between the residents, family members, community nursing team, institutions, and the community partners.

You will be assigned to provide basic health monitoring, preventive health education, health and social care coordination for pre-frail and frail residents in the community. You will also perform basic data management for community nursing services.

Job Requirements

• GCE ‘O’/’N’ level with a recognised Certificate in Integrated Care from the SingHealth Academy or equivalent
• Good knowledge of IT skills
• Good team player, able to work independently and possesses initiative to complete tasks and projects assigned
• Possess pleasant disposition and good interpersonal skills
