Job description

Location: Compassvale AAC

Job purpose

The jobholder is primarily responsible for coordinating referrals & service delivery between SCC, AAC, HT & HPC+ and other relevant services for seniors. The role serves as a key liaison between clients, nursing care assessor and community partners to ensure timely access to medical, social and community-based services.

Duties and responsibilities
  • Coordinate referrals and service delivery, monitor client acceptance, and follow up on service commencement and progress.
  • Act as a client touch point for queries and support from referral to service commencement.
  • Update service availability, referral status, and alert Care Assessors to client condition changes.
  • Build partnerships and facilitate communication across providers and multidisciplinary teams.
Qualifications & Skilled Required
  • Experience in conducting care coordination for seniors
  • Experience in using interRAI assessment & BRIGHT systems; experience in ILTC setting is an
  • Understanding of local eldercare services, grants, and sector workflows; lnterRAI assessment.
  • Proficiency in lnterRAI IT system, BRIGHT, and Community Care Plan system
  • Problem-solving, empathy, cultural sensitivity, client rapport, effective communication, organization, and monitoring skills.
