Key Responsibilities:
  • Review clients’ medical history and care records.
  • Conduct holistic client assessments across medical, functional, cognitive and social domains.
  • Perform standardised assessments (e.g. interRAI, RAF, FAR, MBI, AMT, CFS).
  • Identify care needs and risks using clinical reasoning.
  • Develop and implement care plans with multidisciplinary teams.
  • Coordinate care and link clients to healthcare and community services.
  • Educate and support clients and caregivers on self-care.
  • Facilitate MOH/community schemes and service applications.
  • Advocate for clients’ needs and preferences.
  • Support service improvement initiatives.
Job Requirements:
  • Degree / Diploma in Nursing, Social Work, Counselling, Occupational Therapy, Physiotherapy, or related healthcare disciplines.
  • 2–5 years of relevant experience in acute care, community care, social services, rehabilitation, or care coordination settings.
  • Strong assessment, clinical reasoning, and problem-solving skills.
  • Good understanding of healthcare and community care services in Singapore.
  • Empathetic, client-focused, and adaptable to dynamic care environments.
