Support the care team in coordinating and improving the patient’s care

Work closely with other nursing and allied team to provide advocate care options and services to the patients and their family members

Improve the patient’s care plan by understanding their needs

Educate patients and families to understand how their behavioral health factors can affect their health and illness

Involve in community outreach programmes

Requirements

Degree/ Diploma in Health related fields

Working experience in healthcare or social service sector is an advantage

Possess care management experience
