Job Overview

Service Care Coordination and Case Management within the AWBC Home Visit Programme at KK Women’s and Children’s Hospital.

Key Responsibilities
  • Provide anticipatory guidance, screenings and service coordination for antenatal clients.
  • Collaborate with interdepartmental and external colleagues to optimise health outcomes.
  • Assist with appointment scheduling, rescheduling and client enquiries.
  • Troubleshoot IT issues impacting service operations (e.g. OAS/SAP/SCM/SSNet).
  • Perform health and social screenings (nutritional, mental health, home readiness, safety). 
  • Maintain documentation and complete reporting requirements according to organisational standards.
  • Perform other duties as assigned by Program Lead or Manager.
Qualifications
  • Diploma with 2–4 years’ relevant experience, GCE N/O/A Level, or NITEC in Healthcare.
  • 6–10 years’ relevant experience (alternative).
  • Effective oral and written communication skills.
  • Strong record‑maintenance, organisation and coordination skills.
  • Proficient in word processing, data entry and basic office equipment.
  • Knowledge of supplies, equipment ordering and inventory control.
  • Ability to maintain calendars and schedule appointments.
  • Follow specific instructions and procedures accurately.
  • Deal amicably with all staff and customers.
  • Maintain confidentiality of records and information.
