Responsibilities

  • Conduct pre-admission screening for patients and caregivers.
  • Support nurses in providing holistic care.
  • Identify and report care gaps to the clinical team.
  • Communicate regularly with patients and caregivers.
  • Participate in interdisciplinary team meetings.
  • Maintain accurate documentation of patient care.
  • Coordinate with community partners for continuity of care.
  • Assist in developing care plans based on patient needs.
  • Monitor patients’ vital signs.
  • Provide basic patient education and health advice.
  • Ensure adherence to telehealth monitoring protocols.
  • Coordinate with vendors for equipment and supplies.
  • Participate in quality improvement and risk management projects.
  • Carry out other ad-hoc duties as assigned.

  • Identify and report care gaps, issues, or abnormal findings to the clinical team.
  • Communicate regularly with patients and caregivers, providing updates and handling enquiries.
  • Participate in interdisciplinary team meetings and contribute to care planning.
  • Maintain accurate documentation of patient care and progress.
  • Coordinate with community partners and healthcare providers for continuity of care and referrals.
  • Assist in developing care plans based on patient needs.
  • Monitor patients’ vital signs and escalation red flags to nurses/physicians.
  • Provide basic patient education, lifestyle coaching, and health advice (e.g. diet, exercise, nutrition).
  • Ensure adherence to telehealth monitoring protocols.
  • Ensure availability of equipment and supplies, coordinating with vendors as required.
  • Participate in quality improvement, risk management, and departmental projects.
  • Carry out other ad-hoc duties as assigned.
Requirements
