Responsibilities
- Conduct pre-admission screening for patients and caregivers.
- Support nurses in providing holistic care, including monitoring patient condition and progress.
- Identify and report care gaps, issues, or abnormal findings to the clinical team.
- Communicate regularly with patients and caregivers, providing updates and handling enquiries.
- Participate in interdisciplinary team meetings and contribute to care planning.
- Maintain accurate documentation of patient care and progress.
- Coordinate with community partners and healthcare providers for continuity of care and referrals.
- Assist in developing care plans based on patient needs.
- Monitor patients’ vital signs and escalation red flags to nurses/physicians.
- Provide basic patient education, lifestyle coaching, and health advice (e.g. diet, exercise, nutrition).
- Ensure adherence to telehealth monitoring protocols.
- Ensure availability of equipment and supplies, coordinating with vendors as required.
- Participate in quality improvement, risk management, and departmental projects.
- Carry out other ad-hoc duties as assigned.
Requirements
- Diploma in a health-related discipline preferred.
- Prior healthcare or hospital experience is an advantage.
- Good computer literacy.
- Service-oriented, proactive, and adaptable in a fast-paced environment.
