Assistant Care Coordinator_Dept of Medicine (Contract)

Reporting to the Principal Investigator for the Project, you will be in the role to assist in the coordination for patients, who meet the criterial for screening, to be assigned a slot for Foot and Retina screening.

Job Responsibilities

  • Work closely with the DM Complication Screening Nurses and doctors to set criteria of patient to be screened.
  • Identifying list of eligible DM patients and list of screened patients.
  • Be able to schedule bookings systematically.
  • Be able to understand the current workflow from various medicine specialty in the referral of patients for screening. Then to discuss and set-up workflow with internal stake holders to close the gaps on foot and eye screenings that are done elsewhere.
  • Daily patient statistics and daily patient list are well corroborated on computer.
  • Able to handle enquiries, answer phone calls, record and convey massages independently.

Requirements

  • Diploma in Biomedical Sciences, or Sciences related field, or State-Registered Nurse with diploma
  • Ability to multi-task and work under tight timelines
  • Computer skills in Word and Excel
  • Service oriented and customer focused
  • Be able to work independently when required
  • Excellent communication, problem solving, presentation, negotiation and interpersonal skill
