We are urgently seeking a compassionate and trained Advanced Care Planning (ACP) Facilitator to join Lions Befrienders. The ACP Facilitator will support seniors, family members and care teams in exploring and documenting preferences for future healthcare decisions, ensuring that care aligns with individual values and goals.

Responsibilities
  • Conduct structured ACP conversations with patients and families.
  • Provide information and education on goals-of-care and advance directive options.
  • Collaborate with interdisciplinary teams to ensure ACP documentation is accurately recorded and communicated.
  • Support the implementation of ACP programs and initiatives.
  • Maintain confidentiality and adhere to organizational policies and ethical standards.
Requirements
  • Certified in ACP Facilitator course.
  • Background in degree in nursing, social work, counseling, or related health profession is preferred.
  • Experience in palliative care, geriatrics, or patient advocacy is an asset.
  • Excellent communication, empathy, and documentation skills.
  • Ability to work independently and as part of a collaborative care team.
