Job summary

The Supreme HR Advisory Pte. Ltd. is seeking a Social Service Case Manager for the Buangkok area.

You will coordinate individualized case management, orient cases, and develop intervention goals from onset, reviewing progress on timeline. You will work closely with Probation Officers, Child Protection Officers, families, schools and employers to support reintegration. Requirements include a Degree in Social Work or equivalent and at least 2 years of relevant social service experience.

Qualifications

• Degree in Social Work or equivalent required.
• Minimum 2 years of experience in social services.

Responsibilities

• Individualised case management, including conduct orientation for cases and developing intervention goals from onset and timeline review.
• Collaborate with Probation Officers and Child Protection Officers and other stakeholders including families, schools and employers.
• Develop partnerships with employers, schools and CSO agencies to support reintegration.
• Prepare for, attend and present cases at platforms to update on intervention progress.
• Maintain casework documentation for all records.
• Liaise with Hostel Operations & Development to schedule daily movements.
• Conduct and participate in group work for residents.
• Assist with ad-hoc duties as needed.

Job description

Working location: Buangkok

Working hours: 5 days(Mon - Fri), 9 - 6pm

Salary: $3850 - $5300

Responsibilities

• Individualised case management, include conduct orientation for cases, conceptualise and identify appropriate approaches to develop case intervention goals from the onset and review according to timeline
• Work closely with the Probation Officers (POs) and Child protection officers (CPOs) for co-case management, and also other stakeholders including families, school and employers.
• Develop partnership with stakeholders, including existing and prospective employers, schools and CSO agencies, pro-social communities for reintegration efforts, and community partners.
• Prepare for, attend and present cases at various platforms to update on intervention progress.
• Casework documentation for all case related records
• Liaise with Hostel Operations & Development Department, and provide essential information for Operation (Administration) to schedule for daily movement
• Conduct and participate in group work for residents
• Support in other ad-hoc duties assigned as needed

Requirements

• Degree in Social Work or equivalent with 2 years of experience in social service sector or similar setting
