Job Summary:

Delivering comprehensive medical care to patients of all ages across a wide range of medical conditions. By Promoting Prevention and Screening - providing Improvement in Disease Control & Medication Adherence – Prevention of Disease-related Complications - Fostering Collaborative Care, Referral and Collaboration with multi-disciplinary teams - Enhancing Patient Education, Self-management & Promoting Healthy Lifestyle – Improving Patient Satisfaction and Engagement- Measuring and Evaluating Outcomes.

Primary Responsibilities:

2. Conducting thorough medical assessments, including history-taking, physical examinations, and diagnostic testing, to evaluate patients' health status and diagnose medical conditions.

3. Ordering and interpreting laboratory tests, imaging studies, and other diagnostic procedures to aid in diagnosis and treatment planning based on clinical findings and evidence-based guidelines.

4. Managing the Healthcare of patients with chronic medical conditions through ongoing monitoring, medication management, and lifestyle counseling.

5. Promotes Prevention and Screening emphasizes the importance of prevention and conducting screening tests for individuals at risk of developing disease. Managing the Healthcare of families seeking early prevention & screening through ongoing monitoring, education, and lifestyle counseling.

6. Collaborating with interdisciplinary teams to develop comprehensive care plans.

7. Maintaining accurate and comprehensive documentation reports in medical records, including patient histories, physical examinations, diagnostic test results, treatment plans, progress notes, referral assessment forms & dashboards for population health outcomes.

8. Acting as the immediate point of contact for the patients & navigator of care.

9. Play a crucial role in educating patients about disease management, including self-management & self-monitoring, healthy habits, physical activity, lifestyle modifications and medication adherence. GP would provide guidance on preventing complications associated with diseases and help patients understand the importance of regular follow-up visits.

10. Regularly Monitor and Follow-up his/her group, assess their response to treatment, and make necessary adjustments to their care plans under the supervision of the MRP. Provide regular follow-up appointments to track their progress, address any concerns or complications, and provide ongoing support.

11. Responsible for SPOs Touchpoints achievement & Healthcare Outcome measures in COE group keeping it within the outlined target.

12. New COE Program development in every department with documentation in manuals per program to outline the process & procedures.

13. Support Business Development & promotion of patient’s volume across all COE.

14. Continuous medical education stays up to date with the latest research, guidelines, and treatment options in management. Attending relevant conferences, participating in professional development activities, and engage in ongoing learning to provide the best possible care to their
