A growing healthcare organization in Abha is seeking a compassionate Case Manager to coordinate patient care within a Rehabilitation and Long-Term Care facility. Ideal candidates will have a BSN, a professional license from SCFHS, and 3–5 years of relevant experience.

Qualifications

• Minimum of 3–5 years of clinical experience in Long-Term Care or Rehabilitation.
• Strong understanding of medical, functional, and social needs assessment.
• Familiarity with CBAHI/JCIA standards.

Responsibilities

• Coordinate patient care from admission through discharge.
• Conduct assessments to develop individualized care plans.
• Facilitate communication among healthcare professionals.
