About the Job
We are currently seeking a compassionate and experienced Case Manager to join our team in Abha, KSA.
As a Case Manager, you will be responsible for coordinating and managing patient care within a Rehabilitation and Long-Term Care (LTC) facility.
The Case Manager ensures that patients receive appropriate, efficient, and high-quality care through effective coordination, discharge planning, and communication across multidisciplinary teams.
This role plays a key part in promoting optimal patient outcomes and ensuring continuity of care in compliance with healthcare standards and regulations.
Job Requirements
- Bachelor’s degree in Nursing (BSN) or a related healthcare field — mandatory.
- Must hold a valid professional license from the Saudi Commission for Health Specialties (SCFHS).
- Minimum of 3–5 years of clinical experience, preferably in:
- Long-Term Care and Rehabilitation settings.
- Admission, Discharge Planning, or Care Coordination roles.
Skills & Competencies
- Strong understanding of care coordination, admission & discharge planning, and the continuum of care.
- Ability to assess patients’ medical, functional, and social needs.
- Excellent communication and collaboration skills with multidisciplinary teams (nursing, rehab, social work, physicians).
- Knowledge of CBAHI/JCIA standards related to continuity of care and discharge planning.
- Proficiency in data collection and KPI reporting (e.g., readmission rates, length of stay, discharge delays).
- Proficient in electronic documentation systems (EHR) and reporting tools.
Key Job Responsibilities
- Coordinate and oversee the patient care process from admission through discharge.
- Conduct comprehensive assessments to identify patient needs and develop individualized care plans.
- Facilitate effective communication among healthcare professionals, patients, and families to ensure care continuity.
- Monitor patient progress and adjust care plans to promote optimal clinical and functional outcomes.
- Manage admission and discharge planning, ensuring smooth transitions of care.
- Collaborate with clinical teams to prevent unnecessary readmissions and support timely discharges.
- Collect and analyze data for performance indicators (KPI) such as length of stay, readmission rates, and discharge delays.
- Ensure compliance with CBAHI/JCIA standards and internal policies.
- Participate in quality improvement initiatives and case review meetings.
- Educate patients and families about available care options and post-discharge resources.
