RN Care Manager - Full Time - Eddy SeniorCare (Rotterdam)

Eddy SeniorCare has a rewarding opportunity for a full time RN Care Manager to manage a caseload of patients and help them remain as independent as possible for as long as possible in the community, working with a comprehensive interdisciplinary team.

This position is a community-based HomeCare RN role, providing hands-on nursing care to our participants in their homes.

Work Schedule :

10 hour day shift position, 4 days per week (rotating day off per week)

Requirements :

• NYS RN licensure.
• Two or more years experience.
• Experience and desire to care for seniors.

About Eddy SeniorCare :

Eddy SeniorCare is an exciting, innovative model of care for seniors, located in Latham, 385 Watervliet Shaker Road. We are a Program of All-Inclusive Care for the Elderly (PACE) a unique model that is both the Medicare / Medicaid insurer and healthcare provider giving us maximum ability to customize services to meet participant needs. We care for 140 seniors, age 55 and above who have needs for long term support in the community. As a healthcare provider, we operate a primary care clinic, rehab clinic, a licensed home care agency, transportation service, adult daycare program, home delivered meals, activities, RN case management, social work, and more! This is a truly unique program our region is fortunate to have access to, with only 150 PACE programs across the country!

Pay Range : 36.50 - $49.95

Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
