Case Manger in Tierra de Campos region

Professional who will join a rural care team, as the first point of contact with people in a situation of dependency, providing information, guidance and advice on the range of services offered as well as managing new clients.

Duties

1. Initial assessment of dependent people, identifying what the person needs and provide the necessary support.
2. Draw up the support plan and the financial assessment thereof.
3. Coordinate with professionals from the local public health and social services systems.
4. Monitoring the work of personal assistants.
5. Supervising and monitoring each case.
6. Foster a sense of belonging to the community in coordination with local representatives.

Mainly mornings; flexitime.

Requirements

Hold a university degree in the field of socio-healthcare and/or complementary services; Social Work, Social Education, DUE, Occupational Therapy…
Professionals from the local area preferred.
