Job summary

Creative Health Solutions in Warsaw, Poland is seeking a skilled Physical Therapist to join our therapy team at Skilled Nursing facilities. You will design and deliver rehabilitation programs to improve patients’ physical function and wellbeing, collaborating with nurses and physicians to tailor plans to individual needs.

The successful candidate will hold a State PT license, stay current with CEUs, and document patient progress while communicating outcomes to the care team.

Qualifications

• Licensed Physical Therapist (state license)
• Experience providing rehab in Skilled Nursing facilities preferred
• Ability to collaborate with multidisciplinary team
