Job summary

Mercy Health Home Care and Hospice by Compassus is seeking an IPU Registered Nurse Case Manager who models Compassus values and coordinates care with the interdisciplinary team. The role includes routine assessments, patient education, after-hours on-call support, and ensuring high-quality, compassionate hospice care at home or in facility settings.

The position requires nursing licensure, at least one year of nursing experience, and preferred hospice/oncology/home health background.

Qualifications

• Associate or Bachelor's degree in Nursing required.
• Active RN license in state(s) of employment required.
• Hospice, oncology, home health or long-term care experience preferred.

Responsibilities

• Coordinate clinical, psycho-social and spiritual services per Plan of Care.
• Work with IDT to meet patient and family needs and coordinate care.
• Explain hospice services and Medicare benefits to patients and families.
• Complete documentation at bedside and maintain up-to-date care plans.
• Perform routine and emergency assessments and participate in on-call duties.
• Supervise Hospice Aides and LPNs/LVNs as needed to ensure patient outcomes.
