Qualifications

• Completion of a diploma, associate, or baccalaureate degree in nursing required.
• Current licensure as a Registered Nurse in Tennessee or eligible to practice in Tennessee.
• BLS certification within orientation period and kept current.
• Three years recent nursing experience in acute, long-term, home care, or hospice.

Responsibilities

• Assess home care needs and document assessments and care plans.
• Provide direct care per interdisciplinary Plan of Care to maintain comfort and quality of life.
• Lead and supervise nursing staff, assign tasks by patient acuity.
• Perform ongoing assessments and update care plans with the IDT every 14 days.
