Job summary

eData Services US, L.L.C. is seeking a Remote Telehealth Registered Nurse to support the ALW/CCT programs for California participants via telehealth visits. The RN will conduct nursing assessments, reassessments, monthly monitoring, and care coordination, coordinating with families, facilities, and providers.

The role requires independent work, strong clinical judgment, precise documentation, and HIPAA compliance. Night US hours apply; potential onsite in Taguig as needed.

Qualifications

• Current and active Registered Nurse license in the State of California for at least 1 year.
• Minimum of 1 year of nursing experience required.
• Experience with older adults, disabilities, assisted living, long-term care, home health, hospice, case management, or community programs preferred.
• Experience conducting telehealth visits preferred.
• Knowledge of ALW, CCT, HCBS, Medi-Cal waiver programs, or care coordination preferred.
• Strong nursing assessment and clinical judgment skills.
• Strong documentation, communication, and follow-up skills.
• Ability to work independently in a remote setting.
• Proficiency with electronic medical records, email, phone communication, and telehealth platforms.
• HIPAA knowledge and patient confidentiality.

Responsibilities

• Conduct initial nursing assessments for ALW and/or CCT participants as assigned.
• Complete participant reassessments by telehealth to evaluate health status and needs.
• Assess medical history, diagnoses, medications, ADLs, mobility, cognition, fall risk, skin, nutrition, psychosocial needs, caregiver support, and safety.
• Identify changes in condition or risks requiring further evaluation.
• Recognize when in-person follow-up is needed and escalate as appropriate.
• Document all assessment findings clearly and timely.
• Complete monthly monitoring visits via telephone or telehealth.
• Monitor participant status and report changes, ER visits, hospitalizations, falls, or issues.
• Coordinate with families, facilities, physicians, home health, and care teams.
• Ensure services are provided as planned and follow up as needed.
• Document each visit and follow-up actions accurately.
• Maintain HIPAA-compliant documentation and confidentiality.
