Job summary

A leading healthcare organization in Tarlac City is seeking a Pharmacist Specialist to join their Primary Care team. The successful candidate will conduct medication reviews, focusing on the Home Assessment Pathway. This permanent role requires a Registered Pharmacist with experience in Primary Care and an Independent Prescribing qualification. The position offers 22.2 hours weekly, contributing to patient care across the Moray area.

About the role

We are able to offer a permanent post for a Band 7 Pharmacist with experience of working within the Primary Care setting; the post holder will be working alongside the Home Assessment Pathway Team to review patient’s understanding and need for medication by carrying out Polypharmacy reviews to rationalise their medication regime.

This is an opportunity for a Band 7 to widen their experience and will be ideal for a staff member with existing experience in this area to utilise their Independent Prescribing (IP) qualification.

You will be working across the whole of the Moray area, whilst working closely with the Pharmacy Management Team, based in Elgin.
