JOB SUMMARY: To make certain that all medicines dispensed are 100% free from either error or negligence of the staff.
Duties and Responsibilities
A. Functional Duties
- AMBULATORY DUTIES- the Pharmacist is responsible in giving a special patient information regarding the drug usage, route of administration, as part of total care program.
- IN-PATIENT
- Ensures that the Pharmacist directly review all requisition slips.
- Ensures that established policies and procedures are allowed.
- Checks the accuracy of dose prepared.
- Provides drug control considering proper storage and dispensing.
- Ensures that all states and drug stability and incompatibilities.
- Provides for proper record and billing.
- Maintains professional competence, especially in drug stability and incompatibilities.
- Ensures the new personnel are trained properly in the policies and procedures of the dispensing area.
- It provides a convenient method for instituting drug charges to maintain good accountability.
- Keep the dispensing area neat and clean.
- B. Patient Care Area
- Ensures that record for administered narcotics are kept correctly. The Pharmacist should act as a Liaison between Pharmacist and the Nursing and medical staff communicates with Nurses and physician regarding medication-administration problem.
- Provides Pharmaceutical services at all times to patients, through an adequate supply of safe, effective and good quality drugs in appropriate dosage forms consistent with their needs in collaboration with the medical staff.
- Promotes optimum standers for national drug therapy by establishing a Hospital Formulary.
- Ensures the availability of drugs and medicines for emergency cases and treatment areas.
- Provides the necessary drugs to emergency cases for the first 24 hours regardless of the patient’s capacity to pay.
- Recommends the emergency procurement of drugs not available in the pharmacy.
- Participates in detection, reporting and monitoring of Adverse Drug Reaction (ADR) in the Hospital in cooperation with the PTC.
Requirements
- Educational Qualifications: Bachelor’s degree in Pharmacy or Pharm.D.
- Experience Level: 0–2 years of experience in a pharmacy setting.
- Skills and Competencies: Strong attention to detail and accuracy in medication dispensing.
- Skills and Competencies: Excellent communication and patient interaction skills.
- Qualities and Traits: Compassionate and ethical approach to patient care.
- Responsibilities and Duties: Ability to work effectively under pressure and manage multiple tasks.
