Educational Service District 123 is hiring a Nurse Case Manager for the Migrant Education Program to address health, social, and attendance needs of eligible migrant students across Columbia, Finley, Prescott and Richland districts. Work Days and Salary Options:
Starting Salary: Year-Round (260 Days) Grade 30: $86,824.40 (Step 1/ Year 1) - $97,720.59 (Step 5/ Year 5)

SUMMARY

ESD 123 is looking to hire Nurse Case Manager. Contracted days are negotiable at either full-time year round (260 days) or less than year round (195) days. Benefits including paid leave will be dependent on work days. The Migrant Education Program (MEP), Nurse Case Manager will provide appropriate one-on-one and group nursing case management services to address the combined health, social, and attendance needs of eligible migrant students in Columbia, Finley, Prescott, and Richland School Districts to improve student health and academic engagement. This position will provide health-related education and resources to migrant student and families, to Migrant Education Program and district staff.

ESSENTIAL FUNCTIONS
  • Collaborate with Migrant Education Program and school staff, and community partners to:
    • Identify students who may benefit from services
    • Assess students to identify health conditions
    • Describe how health concerns are impacting student engagement and success
    • Determine if the student would benefit from case management support
    • Prioritize students to include in the case load
  • Manage case load of 40 – 60 students:
    • Conduct an assessment to identify student and family strengths and goals
    • Include student and family to identify student specific interventions to achieve goals
    • Manage referrals to support goals
    • Assist student and family to understand and manage interventions and recommendations from different referrals
    • Coordinate with school staff in development of student plans (emergency care plans, individualized healthcare plans, 504 plans Individualized education plans, etc.)
    • Monitor plan effectiveness and makes changes as needed.
  • Collaborate with MEP to:
    • Identify community strengths and concerns
    • Set community goals
    • Develop interventions to help achieve goals
    • Identify areas of opportunity to serve migrant students
  • Contribute to community events
  • Develop and facilitate education presentations in English and Spanish:
    • Coordinate with community providers to provide support and presentations for community events
    • Understand migrant community and student issues that affect the success of migrant students
  • Document assessment, goals, interventions, and outcomes of work with students, families, community partners and MEP staff.
  • Support inclusion and advocate for the health rights of students and migrant community
  • Provide consultation services to school personnel regarding health issues, e.g., communicable disease control.
  • Serve as a resource for health education to students, families, and MEP program staff
  • Maintain confidentiality and follow all applicable Board policies and procedures

MINIMUM QUALIFICATIONS
  • Current active Washington State Registered Nurse License
  • Current valid Driver's License
  • WSP/ FBI background clearance required
  • Bilingual and Bilterate in English and Spanish
PREFERRED QUALIFICATIONS AND SKILLS
  • Bachelor's degree from an accredited nursing program
  • Two (2) years of experience practicing as a registered nurse
  • Demonstrate experience in one or more of the following areas:
    • School nursing
    • Acute and/or chronic care in pediatrics
    • An independent practice setting, i.e., home care, hospice
    • Computer skills to communicate and collect data
Requires knowledge of
  • Knowledge of professional nursing principles, standards of care, and state and federal obligations
  • Knowledge of current health issues affecting both rural and urban communities, and public schools.
  • Knowledge of Individualized Health Care and Emergency Care plans, Individualized Education Programs, 504 plans, and principles of school health programs.
  • Knowledge of child and adolescent health – physical, emotional, and mental health concepts.
  • Familiarity with regional school health resources and community partners.
  • Ability to interact professionally with people of all ages, lifestyle preferences, and diverse cultural, racial, socioeconomic, and educational backgrounds, including those who may be distraught, angry, or upset.
  • Ability to apply complex nursing, health, and education regulations.
  • Ability to promote equity across all work and interactions.
  • Ability to develop rapport with students and parents, while maintaining professional boundaries.
  • Ability to communicate effectively in speech, writing and professional presentations in both English and Spanish.
  • Ability to handle sensitive situations with tact and objectivity, as well as maintain confidentiality.
  • Ability to utilize health information software programs, technology, and standard office equipment.
  • Ability to work independently with large numbers of staff in promoting the migrant health services program.
  • Ability to practice consistent and punctual.
  • Ability to work flexible hours and make home visits to migrant families as needed.
  • Ability to travel between work sites.
  • Ability to provide first aid or perform CPR when needed.

COMPENSATION
  • Paid once a month on the last working day of the month.
  • Receives an average monthly gross paycheck based on the number of days and hours assigned to the work calendar (Sept 01-Aug 31) and placement on the salary schedule.
  • A full work calendar for this position is 260days and 8hours per day. Below is an example of a gross monthly paycheck for a full work year:
    • 7,051.92(step 1/year 1) -$7,934.25(step 5/year 5)
    • This amount may increase or decrease slightly based on hiring date and months remaining in the work calendar (Sept 01-Aug 31).

WORKING CONDITIONS/PHYSICAL REQUIREMENTS:

Work is performed approximately seventy-five percent (75%) in an office environment and approximately twenty-five percent (25%) traveling to districts and affiliate agencies, as well as for workshops, committee representation, and conferences. Lifts and carries a maximum of thirty (35) pounds. Adequate manual and finger dexterity, hearing, speech, and vision are necessary to perform the essential functions of this position.

Hazards: Those present in a normal office environment; no known significant hazards.
