We’re looking for a skillednurse to join our Integrated Family Health Centre team in Reefton, with aportfolio spanning both district and practice nursing. This is a rewardingopportunity to deliver holistic, high-quality care across all age groups andplay a key role in improving health outcomes for our rural community.

About the role

As a valued member of our interprofessional team, you willdeliver a broad range of primary care, urgent care and community serviceswithin the Integrated Family Health Centre, as well as community-based carethrough home visits. You will play a vital role in supporting the health andwellbeing of tamariki, adults, and whānau by delivering culturally responsive,holistic care to our rural communities.

Key Responsibilities
  • Deliver high-quality care withinprimary care and community settings to enhance health and wellbeingoutcomes.
  • Palliative care, wound care andmedication management in the community.
  • Provide culturally competentcare, prioritising the needs of Māori as tangata whenua to achieve equityin health outcomes.
  • Undertake comprehensive,holistic assessments within culturally appropriate frameworks.
  • Develop collaborative anddocumented care plans that support continuity of care.
  • Work closely with communitystakeholders and the interprofessional team to address unmet healthcareneeds.
  • Foster integration of acute,community, mental health, and primary care within the IFHS.
  • Applycritical thinking to support a culture of safety, effectiveness, andpatient-centred care
  • Ableto work independently as well as collaboratively to ensure the higheststandard of patient care
  • Maintain professional, legal,and ethical responsibilities while ensuring cultural safety.
  • Identify opportunities forservice enhancement and support quality improvement initiatives.
About you
  • New Zealand Registered Nursewith a current Annual Practising Certificate (APC) without conditions thatmay impact role performance.
  • Computer literacy, includingexperience with Indici and Microsoft Office Suite.
  • Strong initiative and ability tothink on your feet
  • PRIME trained or commitment tocompleting training and providing this service to the community
Desirable Qualifications & Experience
  • Previous experience as aPractice Nurse or District Nurse, including managing long-term conditionsand working with standing orders.
  • Current Immunisation andCervical Smear Certificates.
  • Venepuncture
