Job summary

Health New Zealand | Te Whatu Ora is seeking an experienced Individualised Funding Case Manager for the Bay of Plenty region. You will assess needs, develop person-centered care plans, and coordinate services for older adults to support independence.

You will work with whānau, health professionals and community providers to advocate for clients, monitor outcomes, and optimise access to resources. This permanent full-time role offers a collaborative team culture.

Qualifications

• Registration as a Health Professional with a current Annual Practising Certificate.
• Current full driver’s licence.
• Experience undertaking needs assessments and coordinating services.
• Strong communication and relationship-building skills.
• Good computer literacy and experience working with clinical information systems.
• Ability to work both independently and as part of a multidisciplinary team.

Responsibilities

• Develop and review personalised care plans.
• Coordinate services and support networks to meet identified needs.
• Case manage clients with complex needs and Individualised Funding arrangements.
• Work collaboratively with clients, families/whānau, healthcare professionals, and community providers.
• Monitor and review services to ensure positive outcomes and maximise independence.
• Advocate for clients and promote access to available resources and services.
