Job summary

Qualifications

A relevant qualification in health, social services, community development, or equivalent experience.
Strong communication, relationship-building, and organisational skills.
Experience with culturally safe, whānau-centred practice is preferred.
Understanding of Te Tiriti o Waitangi and NZ health system is beneficial.

Responsibilities

Support whānau with tamariki overdue for immunisation and connect them with the right services.
Coordinate with health providers and community partners to remove barriers to care.
Contribute to quality improvement initiatives and equitable health outcomes.
Maintain accurate records and ensure timely follow-up on immunisation plans.
Work collaboratively across organisations to improve immunisation outcomes.
